The bread and butter of family medicine is the relationship of the patient and physician over time. To be able to choose appointment lengths, have timely availability of appointments as well as creative approaches to how care is given (in-person, virtual, or phone. Home visits are possible in some circumstances. We can personalize and be creative with your approach on your wellbeing journey. The membership option means that you are prioritized for timely appointments, have access to education, as well as direct messaging with Dr. Sara. This is a way we can provide exceptional, personal, and direct care.

One-Time Enrollment Fee is $400
Adult Membership is $125/mo
Child Membership (w/adult) is $50/mo

Are you really struggling? Are your medical records a complete mess? You love your primary physician but are confused by your diagnoses, have seen different specialists and complementary care providers and are getting mixed messages - this is the package for you. We will work together to get your medical diagnoses organized, create a binder to be able to take with to appointments and focus education on the parts that are most confusing to you. It is set up to be a high level access to care for a shorter period of time to help you be YOUR best advocate. 6 week (meet every week) and 12 week (meet every other week) packages are available. Also if you are a member or thinking about becoming a member this is also an option and discounts are available in that case.

Dr. Sara will get a notification and she will reach out to you with a special link to schedule an extended first visit as needed. Family coordinated visits are also available. Note that you will not be charged until day of appointment.

Direct Primary Care is just that - You and Your doctor and no one else.
Arise Wellbeing, PLLC is considered a direct primary care and integrative medicine practice. To keep costs low and access to care easy we have chosen to be cash-based. Insurance and Health Systems dictate so much of our current medical care. You will need to verify with your HSA carrier if they will allow reimbursement.
Anyone who's health can improve can benefit from our help. We will work with your to figure out the best way to empower you on your wellbeing journey.
The short answer is no. The longer answer is that in order for me to provide medical care you need to be established as a patient. This requires you to register on-line, and the first visit fee includes the enrollment fee and first month fee at that time which is non-refundable. Patients are able to cancel at anytime after that first month.
Integrative Medicine is the bringing together of conventional medicine and other types of therapies such as nutrition. It is a system of medicine that looks at the whole of the person - mind, body, and spirit in the context of their life. Together we figure out a plan that may include nutrition, herbs, supplements, and pharmaceutical medications.
If you have conventional insurance typically these are covered even though I am not associated with the insurance. However there are exceptions and you are responsible to know the best option for you. I can order at any of the local clinics/labs. I am working on getting discounted cash pay labs and imaging set up at this time.
The short answer to this question is No. I am not serving as an emergency room. However I do believe that many things can be kept out of the ER knowing that I will be able to see you within 1-2 days such as ear pain and sore throats. Of course if you are having chest pain, symptoms of a stroke or have a possible broken bone I would want you to seek care immediately at the nearest emergency room. As a member though remember you can message me and I will be able to see you for quick follow up or if the schedule allows even be able to be communication with you through your ER visit or hospital stay to help you be your own best advocate.
Everyone should have the ability to choose who they want to be their doctor. If you are ready to cancel your membership a few things will occur. One is you have to provide a written cancellation notification. Then your membership will be discontinued. If you want your records to be shared with your next physician you will need to follow their offices' policies and we will forward once a release of information is provided. If you have a fullscript account that will also be discontinued/archived. If there are prescriptions you will be responsible to have your new physician prescribe, after cancellation Dr. Sara is no longer your physician and cannot prescribe for you. If you have outstanding labs pending they may be cancelled or if Dr. Sara receives results she will print and mail them out but would be unable to interpret for you as you would no longer be her patient.
We would love to have you back. In our patient agreement we have a clause that states that from time of cancellation to re-enrollment there is a 6 month wait and you will be responsible for a re-enrollment fee of $400 along with the first month of re-enrollment and your monthly fee will be updated to current membership rates.
